Orthodontics for Children

At our dental clinic in Milan, we treat both adults and children with teeth straightening concerns. Whereas, with adults, braces in Milan are often a cosmetic choice, for children, there are both preventative and dental orthodontic treatments that may be needed.

When should a child first come for an orthodontic assessment?

Whilst orthodontics is often considered to be common in teenagers, we actually recommend that children come for their first assessment between 5 and 6 years old. This is because we can see, at this age, if the deciduous teeth are healthy and if the permanent teeth are present and ready to erupt and how the maxilla and mandible (the upper and lower jaws) are developing.  If there are any major concerns we can offer preventative treatment to ensure that your child’s jaws and teeth develop correctly.



What would happen in an orthodontic assessment at 5-6 years of age?

At this stage we are interested in educating children and parents about:

  • Oral health and hygiene – if there are signs of tooth decay we would flag this up and give some extra instructions on cleaning teeth and prescribe fluoride supplements
  • Oral habits – if your child is a thumb sucker or has mouth breathing issues, we would flag this up and discuss changes to habits to protect the jaw and bite as your child continues to grow
  • Diet – if your child has a lot of decay or dental issues we may discuss diet and reducing sugars to protect your child’s teeth

We are also looking for any signs of orthodontic issues already developing. At this age we are mainly looking for:

  • Any changes or issues with baby teeth that might affect adult teeth
  • Any major occlusal issues – such as crossbite or open bite

It isn’t common to treat orthodontic issues at this age, except in cases where there are health risks.


What would happen in an orthodontic assessment at 7-9 years of age?

The next stage of growth for your child’s teeth and jaws is between ages of 7 and 9. This is where we might recommend interceptive treatments. We would intercept growth issues with the maxilla and mandible, such as anything that is stopping growth of the mandible which may lead to issues with the bite or severe crowding issues.



What would happen in an orthodontic assessment at 11-12 years of age?

This will depend on whether your child has been seen earlier or has had orthodontic treatment. In some cases, there are unique treatments needed at this age to align the teeth and correct any issues with overcrowding. There may also be a second phase of treatment for some children who have already had some orthodontic procedures.



What can you expect from our orthodontic process for children in Milan?

We will monitor your child’s jaws and teeth as they grow – from their first visit aged 5-6 right through to adulthood.

We will also advise on any oral health and hygiene issues and suggest if extra cleaning or diet changes are needed.

When we feel that there is a need for orthodontic treatment, we will explain this fully, giving you a suggested treatment plan. This will also clarify if further treatment may be needed in their teenage years. We will aim to complete treatment that will be as minimally invasive as possible. For example if we anticipate issues with overcrowding, we may suggest earlier treatment to avoid extractions in teenage years.


How long will orthodontic treatment last for my child?

This will depend on the nature and scope of treatment proposed. We will also explain this fully so you and your child can prepare for this. Interceptive treatments aged 7-8 years are often between fifteen and eighteen months long – we aim to keep these treatments short as we understand it can be difficult for younger children. Teenage years treatment are often around eighteen months, if it is unique – this can be reduced if interceptive treatment was also completed.

When orthodontic treatment needs a retention phase this can vary – depending on the unique treatment. We also monitor children for up to two years after treatment to ensure lasting success of the orthodontic treatment.


Adult Orthodontics

Is it common for adults to have braces and orthodontic treatment in Milan?

About one in every three orthodontic cases in our dental office is an adult patient. Adults choosing to straighten their teeth is a more popular choice than ever before. We find that adults choosing braces may opt for the invisible brace systems, such as Invisalign. The speed and discretion of invisible teeth straightening systems has encouraged more adults to choose Invisalign, as opposed to fixed braces.


Why do adults choose to straighten their teeth?



Adults choosing orthodontic treatment often do so for three main reasons:

On average teeth straightening can cost about 4-5000 euros, depending on the system used and the complexity of the case.

We normally have a treatment plan that lasts between 9-15 months. This will then be followed by retainers to maintain the result.

Which system should I choose for my teeth straightening treatment?

Fixed appliances are less work for the patient – your dentist will do any necessary work during appointments. Your job is just to keep teeth clean and to come to regular appointments.

Commonly asked questions when considering orthodontic treatment as an adult

It is perfectly normal to have lots of questions about your orthodontic treatment as an adult. We have compiled some of the commonly asked questions we get asked in Milan – you may well find the answer you’re looking for here!


Can orthodontics fix the gap between my front teeth?

A fixed appliance placed on a few teeth either side of the diastema (the gap between the front teeth) would give the anchorage needed to move the teeth and close the gap. Another option would be Invisalign – which would have the same effect. The most important thing to say is that diastema has a high tendency to reopen so it may be necessary to have a fixed small retainer behind the teeth permanently, in order to keep the teeth in place over time.


Do I just have too many teeth?

Severe crowding can make you feel like you have ‘too many’ teeth but actually the problem is with your jaws being too small for all of your teeth to fit. The solution is to either widen your arches to make space for all of your teeth or to reduce the number of your teeth by extracting some teeth.


Do my crooked teeth need Orthodontics?

You don’t have to correct every occlusion – crooked teeth are not always a bad thing! It’s entirely up to you and if your crooked teeth affects your lifestyle. Some people choose to correct their crooked teeth because:

  • They have aesthetic concerns – this could be a lack of confidence due to your smile
  • They have issues with oral hygiene – crooked teeth can be harder to keep clean and lead to gingival inflammation and deseases
  • They have stability concerns – if your crooked teeth are affecting your bite over time you may want to correct this
  • They have functional disruption – if your teeth stick out this can cause issues with lip sealing and mouth breathing


Am I too old to get my teeth straightened?

In a word, no. The way your teeth react to being moved by pressure from by orthodontic appliances is the same whatever your age, so it is still efficient whether you are 15 or 85!


Are adult braces painful or uncomfortable?

The more uncomfortable option of adult braces is perhaps fixed braces as they are more bulky and require more attention in daily cleaning procedures than the aligners, and involve more adjusting than the aligners, which need to be taken off and replaced. In both cases when the aligners are first changed or the fixed braces tightened this can cause some discomfort but only for the first couple of days each time, as the teeth move most in this period.

Speech shouldn’t be affected much with either brace system, apart from the first few days where it may take some time getting used to the teeth straightening system in the mouth.


Am I more likely to get tooth decay with adult orthodontics?

It is uncommon to see increased tooth decay as patients are shown how to effectively clean their teeth with fixed braces. Also the brackets attached to the teeth are fixed with a resin that releases fluoride, helping to prevent decay. We asses plaque build up during checkups and if needed will refer to the hygienist for advice on thorough cleaning and instructions.


How do I know if I have an open bite or cross bite and how does this affect my teeth and mouth?

An open bite is where the upper and lower front teeth do not overlap. If the teeth do not overlap properly at the back then this is called a posterior crossbite.

Finally an anterior crossbite means the lower jaw is protruding out at the front. This is one of the few treatments completed at a very early age as if not corrected, prevents the upper jaw from growing properly.



Will adult braces affect my lifestyle?

The only change is that you may need to dedicate a bit more time to your oral hygiene routine and appointments at the dentist. You can eat and drink as normal and can talk normally as well. The only effect it will have is the positive one when you have your straighter smile after treatment and increased confidence in your new smile!


The health benefits of orthodontic treatment

While some people may choose teeth straightening for the aesthetic improvement of the appearance of their teeth, there are also health benefits to orthodontic treatment.


What are the health benefits of having straighter teeth as an adult in Milan?

Straighter teeth are easier to keep clean and so can improve your oral hygiene. Aligned teeth are also functionally more healthy.

This is because the load of the occlusion is properly distributed: if there is more pressure on one side of the mouth or the other this can lead to problems over time, including loss of bone and gum recession – straighter teeth avoid this. Finally there are also health benefits with breathing and straighter teeth – crooked teeth can sometimes stop the lips from closing properly, causing mouth breathing.


Can adult braces improve your gum health? 

Crowding and crooked teeth can make cleaning more difficult which can lead to oral health issues, such as inflammation of the gums when plaque builds up. By straightening the teeth you are promoting oral and gum health.


Can closing gaps between your teeth help your oral health?

When you have gaps between your teeth you can end up chewing on your gums, with food staying trapped in the gaps between your teeth. Teeth are also more stable when the gaps between them are closed.

When you choose teeth straightening at our dental office in Italy, your dentist will outline the costs of treatment before you commit to the treatment plan. There are three main factors that affect cost of your orthodontics:

  • What appliance is to be used? – if you only need a limited treatment with less appliances this may be cheaper
  • What system is to be used? – we offer a range of the teeth straightening systems, some of which may have a higher cost than if you choose traditional metallic brace
  • How long will treatment last? – the number of appointments and the total length of treatment will affect the cost
  • How complex is the treatment needed? – the more complex and the more clinical time and expertise, the more the treatment will cost

Can I spread the cost of treatment?

We understand that adult braces are an investment. We split the cost into trimestral payments.

We also charge an initial treatment planning fee, to cover the cost of the comprehensive planning of treatment. The initial payment is slightly higher followed by a number of equal payments every three months throughout your treatment period. We will then give you a quote for the retention phase.

What to expect while you are undergoing orthodontic treatment in Milan?

Contacting the dental office

The dental office secretary will be your first contact, either by phone or in person. They will book an appointment and ask you to bring any x-rays or paperwork you may have.



The First Appointment

You will be greeted by our secretary who will ask you to complete some simple forms for your medical dental and orthodontic history.

You will then meet your orthodontist, who will go through the paperwork with you to ensure all of the information needed is in there. The next step is the clinical exam.


The Clinical Exam 

This begins with examining oral hygiene, looking for signs of plaque, decay and any need for hygiene instruction.

The next step is to look at the occlusion – do you need orthodontic treatment? If so, we will ask you to schedule an appointment to take full your orthodontic records, which is necessary to make a diagnosis and prepare a treatment plan

We may ask you some more questions at this time just to get a more comprehensive picture of any issues with mouth breathing or occlusion.


How do I prepare for my first appointment?

Make sure you bring with you any previous x-rays or any details of orthodontic treatment you have had in the past. It also helps to be able to clearly explain the goal you have in mind for treatment. Sometimes patients make notes and also note down any questions they want to ask.


Taking your records for Orthodontic Treatment

This will be in another appointment where we take x-rays and photos of the face and teeth and also an impression of the teeth. We may also use an intraoral scanner to get a digital picture of each tooth.

In the following appointment you will be presented with a treatment plan which will include an accurate cost quotation for treatment. You will be able to ask any questions and find out about payment plans.

Once you have agreed to the treatment plan we can book your first appointment to begin treatment.


What happens after Orthodontic treatment is completed in Milan

Once your teeth have moved into their new position, we will then plan the removal of the appliances. With both fixed appliances and Invisalign we will need two appointments, usually scheduled on two consecutive days. The first one will be for the removal of appliances or the attachments, picture and impression taking for retainers. The second appointment will be for the fitting of the retainers. Your teeth will feel and be loose when your braces are removed. This is totally normal.

A removable retainer will be made for every arch that has had teeth straightening. You will need to wear this for a few months, depending on the type and extent of the movement of the teeth. This will then reduce to wearing at night, and then every other night. You will have regular retention appointments to monitor the stability of your orthodontic treatment.

It’s really important you follow the instructions given in the retention period to ensure your teeth stay in their new straighter position.
